Thank you for your interest in the in motionTM Community Challenge evaluation study.
If you choose to participate in the study, you will provide us with valuable information regarding (a) who is participating in the in motionTM Community Challenge, (b) how challenge members thoughts, feelings, and behaviors change as a result of the Community Challenge, and (c) what you liked about the Community Challenge and how you think it can be improved for future years. We request that all participants in this study complete three surveys that will take 10-20 minutes to complete, including a baseline survey before you begin the in motionTM Community Challenge, a follow-up survey after you have finished the challenge (a link to the survey will be sent to you on November 1, 2016), and a second follow-up survey that will be sent to you April 1st, 2017. If you have already started the challenge but you are interested in participating in the study, we would really like to hear from you as well, so we strongly encourage you to participate. Involvement in this study is completely voluntary, and you are not obligated to answer any questions you don’t feel comfortable answering.
By participating in the study, you will have the opportunity to be WIN up to 3 draws for Fitbits®. The draw for the first FitBit® will occur by the end of October, 2016 for participants of the first survey, the second by the end of November, 2016, for participants of the second survey, and the final one will be drawn by the end of April, 2017 for participants of the third survey.
